To prepare for your appointment, we suggest that you print our downloadable Registration Forms (see left side of page), fill them out completely, and sign them. You may fax the completed forms to us or bring them to us. This will speed up the mandatory processing of your file and you will get to see the doctor much more quickly.

If you are a returning patient with a chronic illness such as hypertension or diabetes, you may find the blood pressure log form and blood sugar log form charts useful for monitoring your condition. Please print the attached Prescription and OTC medication log - fill it up with pencil and keep it updated, in your wallet.

Remember, the appointment is scheduled for your benefit so please come prepared and do not hesitate to ask questions. We recognize how valuable your time is and want to help you make the most out of it.

Please make sure to do tests ordered on a timely manner to help diagnose or monitor your medical condition. Remember- No news is NOT good news. In many cases, phone communication may not convey the same explanation of tests like a face to face appointment would. Please make sure to keep your follow up appointments to discuss results if advised so.
